All Smiles hours of operation are 7:00 am until 6:00 pm Monday through Friday.
Parents are welcome to visit All Smiles at any time during the hours of operation.

Health Inventory Form
First page of the Health Inventory
 Form must be completed by the Parent and/or Guardian. The second page of the Health Inventory Form must be completed by your family Pediatrician. All children are required to have a copy of their current Shot Records and Lead Form.

Emergency Form
Please make sure that all information such as phone numbers are current and working. If there is another adult that you will assign for pick up please attach an additional sheet listing the individuals. We will not release a child to individuals not listed on this form. NO EXCEPTIONS If the individual is new and on the form, ID will be required.​ *Note: Documents are updated yearly
Medication Administration Form
Effective medication management begins with having your child's documents properly filed. Please be sure to complete a medication administration form for all oral and topical medications. *Note: Parent consent is required to be documented before medication is administered.
Asthma Action Plan/ Medication Authorization Form
Do you have an action plan in place in the event of an emergency for your child's asthma? Please be sure to review the asthma medication authorization plan. *Note: This form must be completed by a medical provider to authorize administration of asthma medications.

Music is used during transition for breakfast. Children use the restroom.

Teacher Directed Activity (Group Activity)
8:45 AM – 9:00 AM
Math and Manipulative's. Lessons incorporate a variety of delivery styles to meet the diverse learning styles allowing children in care to work with other classmates in small groups focused on targeted interests and /or one-on-one with provider. All lesson and materials incorporate differentiated instruction to meet each child at their point of need.

Children begin to experiment with sounds; move in response to music and rhythm.
Language / Reading Readiness

Lessons incorporate a variety of delivery styles to meet the diverse learning styles allowing children in care to work with other classmates in small groups focused on targeted interests and / or one-on-one with provider. All lesson and materials incorporate differentiated instruction to meet each child at their point of need. 15 minutes of reading included.

Outside Play /Gross Motor

Children move and engage in a variety of activities that promote an active lifestyle. There are planned activities as well as the opportunity for free choice.

Children transition to lunch through verbal prompts and music. Children use the restroom.

Free Play/Parent Pickup-Departure
5:15 PM – 6:00 PM
As pick-up time approaches for children, each child is prompted to prepare for dismissal by cleaning up, gathering their belongings and saying good-bye to friends.
